Cool Bed PLUS Coordinator
East County Communities Center
Full-Time: $32,000 per year plus excellent benefits
Specific Responsibilities: Accountable to Program Manager. This Coordinator will manage all of the incoming referrals and screen out who is eligible before referring to Cool Bed PLUS Program. The Coordinator will be responsible for maintaining client case files with complete and up to date records. Responsible for the development and coordination of an emergency response, child abuse prevention service consisting of: short term crisis intervention, access resource and volunteer shelter for youth in SDYS certified foster homes.
Summary of Duties:
- Adhere to and uphold the rigorous standards of SDYS as mandated by mission, purpose, and guiding principles and standards of quality. Participate in their refinement.
- Acquire knowledge of the role and responsibility of the SDYS Board of Directors and stay current.
- Participate and implement volunteer system as appropriate. Supervise and evaluate volunteers as assigned.
- Assist in updating agency policies and procedures. Carry agency big picture and stay current on agency activities.
- Maintain and supervise four voluntary foster beds.
- Maintain positive daily relationship with Foster Parents, ensuring the follow through of Community Care Licensing regulations and guidelines.
- Work with the Sheriff Department.
- Assess for mental health needs on all intakes and referrals.
- Communicate with Foster Parent and Clinician regarding special needs or mental issues/concerns on a daily basis.
- Establish a community based referral system in order to identify and provide services to families who are at risk for runaway, homelessness, or child abuse.
- Provide, or cause to be provided, a 24-hour emergency response to families.
- Identify and/or develop community-based resources to enhance program services, and healthy activities necessary for the preservation of families beyond the scope of this program’s services.
- Coordinate all services rendered to service-partner during program enrollment and develop an after-care plan.
- In partnership with family, develop a comprehensive youth and family service plan.
- Provide youth and families with mediation, conflict resolution and problem solving strategies.
- Maintain complete and accurate files on each youth in the program.
- Coordinate and provide for transportation needs of youth in the program.
- Provide assistance with outreach to the community.
- Supervision of the youth while in the center.
- Dialogue and coordinate with the Schools throughout the youth’s placement.
- Reporting Monthly and Quarterly Statistics.
- Maintain confidentiality and privacy standards of service partner protected health information and other applicable information and material in accordance to agency and center policies, and applicable regulations and laws including HIPAA.
- Participate in implementing agency’s resource development plan as appropriate.
- Maintain warm, welcoming office grounds and facilities.
- Maintain a working knowledge of MIS equipment.
Qualifications: BA in social work or related field. Two years experience in working with adolescents. Experience in crisis intervention, youth participation and youth development. Ability to obtain certification in CPR and First Aid. Ability to work with schools, Law Enforcement, social service professionals, and parents of diverse backgrounds. Good driving record and willing to use own properly insured car for the job (mileage reimbursed). Bilingual (Spanish) preferred. Employment offer will be subject to clearances which include fingerprinting, background checks, and drug testing. Must also be able to obtain clearance to work with the Sheriff Department.
