Case Manager
Teen Options
 

Specific Responsibilities:  Accountable to the Program Manager. Responsible for maintaining an active caseload of pregnant and/or parenting service partners, and supervision of milieu operations. 

Full-Time:  $31,000 per year plus excellent benefits 

Duties:

  1. Adhere to and uphold the rigorous standards of SDYS as mandated by its mission, purpose, guiding principles and standards of quality.  Participate in their refinement.
  2. Acquire knowledge of the role and responsibility of the SDYS Board of Directors and stay current.
  3. Assist in updating agency policies and procedures. Carry agency big picture and stay current on agency activities.
  4. Participate in implementing agency’s resource development plan as appropriate.
  5. Participate and implement volunteers/interns system as appropriate. 
  6. Responsible for the facilitation of project and contract activities, parenting classes and other psycho-educational classes on site and off-site as required.
  7. Maintain data collection and contract compliance of records as required.
  8. Compile documentation that will support all reporting and evaluation requirements.
  9. Develop and maintain linkages with community referral and recruitment source.  Provide outreach presentations as assigned.
  10. Maintain accurate and timely records for each service partner.
  11. Participate in file reviews.
  12. Attend required meetings but not limited to supervision, staff meetings, trainings, professional collaborations and community projects.
  13. Assist in the development and maintenance of the Teen Options curriculum.
  14. Provide case management services to service partners as outlined in the contract.
  15. Assist with the supervision of clients while on site.
  16. Assist in maintaining the upkeep and appearance of the program facility on a daily basis.
  17. Provide transportation and other services to assist youth in reaching their goals.
  18. Maintain confidentiality and privacy standards of service-partner health-protected information and other applicable information and material in accordance to agency and center policies, and applicable regulations and laws including HIPAA.
  19. Maintain a working knowledge of MIS equipment.
  20.  

Qualifications:  Bachelor's degree in social work or social service related field with minimum of two years experience preferably in working with adolescents and infants in alcohol or other drug setting.  Knowledge of child development.  Willingness to obtain state approved AOD counselor certification. One year of experience in state licensed daycare facility.  Experience in crisis intervention group facilitation, documentation, and case management.  Must be a team player with ability to work independently.  Good driving record and willing to use own properly insured car (mileage reimbursed). CPR/First Aid Certified.  Prefer bilingual Spanish/English.  Employment offer will be subject to clearances which may include fingerprinting, background checks, and drug testing.  Hire is pending the results of appropriate clearances.
